About our practice
At the Palmetto Health-USC Family Medicine Center, we offer complete health care for children and adults, including obstetrical care for expectant mothers. Our caring team of physicians, family physician assistants and psychologists provides excellent comprehensive care, and we also have certified diabetes educators and a pharmacologist on staff. We are integrated with the University of South Carolina School of Medicine's Department of Family and Preventive Medicine in Columbia, South Carolina, and combine the convenience of a community-based practice with the technology and resources necessary to treat a broad range of illnesses and injuries.
We understand that keeping people healthy goes beyond our role of diagnosing and treating acute and chronic illnesses, which is why we put a strong emphasis on health maintenance, disease prevention and chronic disease management. Focused on innovative care models, we have incorporated many elements to make the patient experience better for the patients we serve. In addition, we emphasize patient education through our classes, lectures and public service activities.
Patient-Centered Medical Home
Our practice was the first Level 3 medical home in the state of South Carolina to be recognized as a Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA). As a Patient-Centered Medical Home, we are a health care setting that emphasizes partnerships between patients and their personal physicians, and their families, when appropriate.
Recognized by the American Heart Association
Family Medicine Center has achieved the American Heart Association (AHA) Gold Level Award for blood pressure control. This national award recognizes practices with an overall hypertension control rate of at least 70 percent for their gold level recognition program. Nationally, AHA had 340 Gold Level winners with 152 from South Carolina.
